Citizen Services - Change of Name
- Embassy ID;
- Birth Certificate;
- Two (2) Passport size photo;
- Affidavit for change of name;
- Written application for change of name;
- Marriage, Divorce or birth certificate (if applicable);
- Photocopies of Nigerian Passport (data page and page 1-5);
- Proof of address (recent utility bill, not more than 3months old);
- Proof of newspaper advert in Nigeria, where the name change advert was placed.
