Passport - Endorsement
- Letter from school and GP;
- Proof of address (utility bill);
- Letter of consent from the father;
- Two (2) passport sized photograph;
- Photocopies of child birth certificate;
- Photocopies of mother’s passport (data page);
- Applicants for endorsement must be aged from 1month – 5years.
